Are you at risk of pancreatic cancer?

The initial part of this questionnaire is anonymous and will help you determine if you are in a group of high-risk individuals. Completion is fully optional and anonymized data will be kept for statistical purposes. If it turns out that you belong to a high-risk group, and you have no other restrictions for clinical follow-up, we will offer you the opportunity to be put on a waiting list to participate in the clinical performance study of the LIPIDICA test, which aims to gain approval of the method for clinical use by regulatory authorities.

LIPIDICA study questionnaire

To evaluate the questionnaire, you must select YES or NO for each question.

Have at least two of your first-degree relatives (parent, sibling or child) and/or second-degree relatives (grandparent, aunt, uncle, grandchild, niece, nephew) been diagnosed with pancreatic cancer?

Have you been diagnosed with at least one of the following: Peutz-Jeghers syndrome, Familial Atypical Multiple Mole Melanoma syndrome, hereditary pancreatitis?

Have you been diagnosed with at least one of the following diseases: Lynch syndrome, Hereditary Breast and Ovarian Cancer syndrome, Li-Fraumeni syndrome, familial adenomatous polyposis?

Are you over 18 years old?

Are you willing to sign an informed consent explaining the risks associated with participation in the study?

Have you been diagnosed with cancer (other than melanoma) in the past?

Do you follow a vegetarian or vegan diet?

Are you a pregnant woman?

Are you willing and able to undergo an MRI (magnetic resonance imaging) scan?

The examination is not possible if:
- you have an MR incompatible pacemaker
- you have an implantable cardioverter-defibrillator (ICD)
- you have a neurostimulator or a cochlear implant
- you have metal fragments in the orbit (around or in your eye)
- you have had an allergic reaction to a contrast agent in an MRI scan
- you suffer from claustrophobia (fear of being in closed spaces)
- you have metal MR incompatible implants (stents, coils, clips, osteosynthetic material) inserted less than 6 weeks before the examination

Are you willing and able to undergo EUS (endoscopic ultrasonography) imaging?

Possible limitations include: a post gastrectomy condition, high gastric resection, oesophageal obstruction.
